How to get a passport in Arizona?

First Time Passport
The First Time Passport applies if:
- This is your very first U.S. passport, or
- Your last passport was issued over 15 years ago, or
- Your last passport expired and was previously issued while under the age of 16.

Lost, Stolen, or Damaged Passport
You can apply for a passport replacement when you can prove your valid passport was lost, stolen, or severely damaged. Except for adult passport renewals that use the DS-82 passport application form, all other applications require the DS-11 application to be completed. Once you have filled this out, leave it unsigned and print two copies. Take these documents to your local post office, along with proof of your identity, United States citizenship, and 2” x 2” passport photographs on a white or off-white background. Separate payment is required to the U.S. Department of State for their passport fees, which start from $60, up to $170 for expedited applications.
