First Time Passport

The First Time Passport applies if:

  1. This is your very first U.S. passport, or
  2. Your last passport was issued over 15 years ago, or
  3. Your last passport expired and was previously issued while under the age of 16.

Passport Renewals

Passport renewals for children under 16 are not as straight-forward due to increasing fraud and applications for child passport renewals can only be done in person when applying at the U.S Department of State.

Application Requirements


Except for adult passport renewals that use the DS-82 passport application form, all other applications require the DS-11 application to be completed. Once you have filled this out, leave it unsigned and print two copies.  Take these documents to your local post office, along with proof of your identity, United States citizenship, and 2” x 2” passport photographs on a white or off-white background. Separate payment is required to the U.S. Department of State for their passport fees, which start from $60, up to $170 for expedited applications.

There are post offices in Arizona with passport acceptance facilities and some may require an appointment.
