There are different kinds of passport applications and the documents you need must align with your choice as follows:
A. New Adult Passports
These passports apply to adults who have not had a passport before. Here is what to bring for passport appointment if you fall in this category:
- A complete DS-11 form: This passport application form includes details about the applicant. You should fill it out but do not sign it until a local acceptance agent has verified your documents.
- Proof of identification: You can use documents like passport cards and driver’s licenses for this.
- Proof of U.S citizenship: You must submit evidence of your citizenship through documents like birth certificates or consular reports of birth abroad.
- Two passport photos: The pictures should be recent and must comply with the U.S passport photo requirements.
You should verify the documents at a local acceptance agent before signing your DS-11 and submitting the documents to a state passport office.
B. Minor Passports
Anyone under 16 qualifies for this passport type. The applicant must submit all the passport requirements under adult passport applications above. Moreover, the application should include the following:
- The parent’s ID: The parent or guardian should include their ID and a copy in the application.
- Proof of citizenship and identification of the parent, e.g., a passport book.
Parental consent is also important. The parent or guardian must appear during the passport appointment. If this is not possible, the parent must submit notarized authorization.
C. Passport Replacements
If you have reported your passport lost or stolen, you will qualify for a new passport. You must submit all the documents under new adult passports and include a complete DS-64 form.
D. Passport Renewals
People whose passports have expired can apply for a renewal from the state passport offices. Here is what to bring to passport application:
- A complete DS-82 form: Complete the form and sign it. Unlike a DS-11, a local acceptance agent does not need to verify your papers.
- Your most recent passport: You don’t need to present proof of U.S citizenship or proof of identification if you have your previous passport. It will serve this purpose.
- Two recent passport photos.
