- It must have been issued 15 or fewer years ago,
- It must have been issued in your current name: There are two ways to go about this. But if you lack documentation for this legal name change, you will need to get a new passport in your current name.
2. A Complete DS-82 Form: You must complete the DS-82 form with accurate information. If you often travel, be sure to get the 52-page book, as this will allow you to have enough blank pages for your subsequent journeys without having to renew your passport,
3. Name Change Documents: This section only applies to people who have changed their names since the last issue of their current passports. You will need to provide supporting documentation regarding the name change. Acceptable documents include court-ordered name change documents, marriage certificates, and divorce decrees.
4. Passport Photographs: Provide two colored and recent passport photos.
You will then pay the fees for your passport renewal and include these when sending your application to the state passport offices.
